5. Fill in:
   - LLC name: Palace Fund LLC

   - Business address
   - Registered agent name and address
   - Manager/member information: Junwon Park
6. Pay $20

## Ongoing

After the initial filing, this must be filed every **2 years** by the end of the anniversary month of formation (February).

## Notes

- The initial Statement of Information is due within 90 days of formation — this is the first one
- Subsequent filings are every 2 years
- Failure to file: $250 penalty and potential suspension of the LLC
